[NeXus-committee] suggestion: remove attribute 'index' from NXDL element <dim>

I suggest to remove the attribute 'index' from the
NXDL element <dim>.

Rationale:

It's redundant. There is no reason not to assume that
<dim> elements within one <dimension>...</dimension>
clause are in natural order.

Implementation:

The new version of utils/nxdl2rst.py will check the
values of 'index' for consistency. This check can
be easily removed once the attribute 'index' is
declared obsolete.

Removal from the *.nxdl.xml files: same question
as in my previous suggestion.
