Comment (by Pete Jemian):

 The XSLT transform {{{nxdl2docbook.xsl}}} generates a URL with an explicit
 href link.  For this to work in all forms of the finished documentation,
 the code should not generate a URL but rather DocBook links and references
 in the right places.  Then DocBook can sort out how to make the URLs for
 the all-in-one html or the separated pages html.
