 Tried just installing a new hyperref package into the existing TeX
 installation on my Mac (into /sw/share/texmf-dist/tex/latex/hyperref) and
 then running texhash.  This path had additional dependencies to overcome.

 Instead, solved the problem by installing the latest MacTexLive into a
 separate directory and then placing the /usr/texbin directory onto the
 path ahead of /sw/bin.

 On the Mac, this solved the problem.

 Suggest adding or updating TexLive on the nexusformat.org server might be
 the way to solve the problem.
